Accessing Your Medical Record

If you are a patient of Warner Hospital & Health Services, you can access your medical record through the patient portal, MyHealth. This online tool gives you access to your health information from virtually anywhere at any time.

If you have an iOS or an android phone, the Meditech app is also available for your use. First search for the Meditech MHealth app. Then search for the Warner Hospital MHealth location. Choose that portal and you will then be directed to the sign on screen. It makes it a very easy and convenient option.

We are also able to offer you the ability to access your health information using a web-based application of your choice to pull all your health information from multiple portals into one secure place.

As a patient of WHHS, enrolling MyHealth will allow you access to:

  • Review Lab and Radiology Results
  • View Medications
  • Request Updates to Personal Information
  • See Visit History and Discharge Information
  • View Upcoming Appointments
  • View, Print, or Transmit Medical Information
  • View Billing Information
  • MyHealth is completely secure, so you can be confident that private information is protected.

How to get signed up?

If you are interested in obtaining access to your MyHealth patient portal, please fill out the Patient Portal Enrollment Request form listed below and return to the Health Information Management Department.

Upon receipt of a completed Patient Portal Enrollment Request form, the Health Information Management Department will process your request and within five (5) business days you will receive a welcome e-mail with a one-time user ID and password. This one-time user ID and password will expire within 72 hours. The Health Information Management Department staff may contact you if there are questions in regards to your enrollment request.

To obtain copies of your medical records please contact the Health Information Management Department. HIM complies with the HIPAA Privacy standards and will not release your private health information without the proper authorization. The Warner Hospital & Health Services Release of Information Authorization including Family Medicine is available for download. You may complete the authorization and fax to 217-935-4928 or email to himquality@warnerhospital.org. If you need to speak to us directly please call 217-935-9571  ext. 3305 for hospital records or x3307 for Family Medicine records.

Health Information Management hours are Monday – Friday from 7:00 a.m. to 4:00 p.m.
